DDH Information Record Booklet
This DDH information booklet has been designed to help caregivers keep a record about their child’s DDH journey. We recommend it's taken to every appointment and help is sought to fill it out.
The booklet was developed to help you remember the information discussed verbally in appointments. We know that recalling what is said can be hard, especially when you are tending to your child at the same time. It is not designed to be a part of a child’s formal medical record.
We have made this booklet free for all to use. If you like what we are doing please show your support by donating today so we can expand on these services.
Healthy Hips Kits For DDH Kids
Our Healthy Hips Kits for DDH Kids include the record booklet. Launched this month, we are making these information packs available, to service providers, for families of children newly diagnosed, to help make their early days of treatment a little less overwhelming. Thanks to the support of the initiative sponsors, Merino Kids, Starfish Babes and Taylah Tree, we are able to offer these packs for free. Please help us ensure all new DDH families receive these by telling your specialist about the initiative today and ask them to contact us for more information.
We have also made the DDH information record booklet available for download here by families already undergoing treatment. Once you have filled the initial booklet be sure to download the additional inserts so you can continue recording the review outcomes in one place.
